SACOT vs. IOP vs. Residential

If you or a loved one is facing a substance use disorder, one of the first and hardest questions is where to start. Anuvia offers several levels of care, and the right one depends on the severity of the substance use, whether a mental health condition is also present, and what kind of daily structure and support will actually work for your life.

This guide breaks down three of Anuvia’s core treatment options: Intensive Outpatient Treatment (IOP), Substance Abuse Comprehensive Outpatient Treatment (SACOT), and Residential Treatment. Understanding how they differ can make the decision feel less overwhelming and help you or your family member get connected to the right level of care faster.

Intensive Outpatient Treatment (IOP)

IOP is designed for people managing serious substance use disorders who don’t require 24/7 monitoring. It’s structured enough to create real accountability and skill-building, but flexible enough to work around a job, school, or family responsibilities.

What it looks like:

  • Meets three times per week for 12 weeks
  • An optional Continuing Care phase, meeting once weekly for four to six weeks
  • Both morning and evening sessions, so participants can keep their regular routine
  • Group therapy, individual therapy, and support group involvement (including 12-step programs like AA and NA)
  • Relapse prevention planning built around each person’s specific triggers
  • Available in person or via telehealth for added flexibility

Who it’s for: IOP works well for people with moderate to severe substance use who have a stable home environment and enough support to apply what they’re learning outside of sessions. It’s often the right fit for someone who doesn’t need medical detox or round-the-clock supervision but does need more than a weekly check-in.

Substance Abuse Comprehensive Outpatient Treatment (SACOT)

SACOT sits a step above IOP in intensity. It was built for people dealing with more entrenched patterns: severe substance use, a history of relapse, and often a co-occurring mental health condition like depression, anxiety, or PTSD.

What it looks like:

  • Daily structured programming, using the Integrated Dual Disorders Treatment (IDDT) model to address substance use and mental health at the same time
  • Substance use education and mental health education
  • Group therapy
  • Case management to help with housing, employment, and other practical needs
  • Weekly family sessions

Who it’s for: SACOT is a strong fit for someone who has already tried a lower level of care and relapsed, or who is managing a diagnosed mental health condition alongside their substance use. The daily structure offers more consistent support than IOP without requiring a full residential stay.

Residential Treatment

Residential treatment, also called inpatient treatment, is Anuvia’s most intensive level of care. It removes someone from their day-to-day environment entirely so they can focus on recovery without the pressures of work, home, or a living situation that may be working against them.

What it looks like:

  • Care available 24 hours a day, seven days a week
  • Length of stay ranges from one to 45 days, based on individual need
  • Medical monitoring for detox, when needed
  • Individual and group therapy for both substance use and mental health, using the IDDT model
  • Educational sessions on how drugs and alcohol affect the body and brain
  • Family sessions
  • Medication management
  • Aftercare and relapse prevention planning before discharge

Who it’s for: Residential care is often recommended for people with severe or long-term substance use, those who need medically monitored detox, or anyone whose current living situation makes outpatient treatment genuinely unsafe or unworkable. A drug screen is not required for admission. Anuvia’s team understands that many people entering residential treatment are actively using, and meeting people where they are is part of the approach.

How to Decide Which Level Fits

A few questions can help point toward the right starting place:

Has this been tried before?
If a person has completed IOP or a similar outpatient program and relapsed, a more intensive level like SACOT or residential care may offer the additional structure needed this time.

Is there a co-occurring mental health condition?
SACOT and residential treatment both use the IDDT model to treat substance use and mental health simultaneously, which tends to produce better outcomes than treating either one alone.

Is the home environment supportive?
Outpatient programs like IOP and SACOT depend on having a stable place to return to each day. If home life is chaotic or actively contributes to substance use, residential treatment offers a safer, more controlled setting to start recovery.

How much daily structure is realistic?
IOP meets three times a week. SACOT meets daily. Residential is around the clock. Matching the level of structure to what someone actually needs, without over- or under-treating, matters for long-term success.

These Levels Often Work Together

Choosing a starting point doesn’t mean staying there for the rest of treatment. Many people begin in residential care to stabilize, then step down to SACOT for daily structured support, then move to IOP as they build independence. Others start directly in IOP and only move to a more intensive level if they need additional support along the way. The goal is matching the right level of care to where someone is right now, not locking into one path permanently.

You Don’t Have to Figure This Out Alone

The right level of care isn’t always obvious from the outside, and it doesn’t have to be a decision you make by yourself. Anuvia’s clinical team conducts an assessment for every incoming client, and that assessment, not a guess made beforehand, determines which program fits your specific situation, insurance coverage, and goals.
